Thank you for your thoughtful intention to make a gift of securities to the Pen Bay Healthcare family of services, which includes the Penobscot Bay Medical Center, Kno-Wal-Lin Home Care and Hospice, Anderson Inn at Quarry Hill and the Knox Center for Long-Term Care.

 The instructions for making an electronic transfer of securities or for presenting physical stock certificates are listed below. If your brokerage account is with a firm other than UBS Financial Services; your brokerage firm may electronically transfer the shares. Please provide your broker with the following information:

TRANSFER OF STOCK CERTIFICATES

If you have in your possession the physical stock certificates, you may bring them to UBS  Financial Services at 30 School Street, Second Floor, Rockland, Maine, 04841, to be deposited into the Pen Bay Healthcare account.
