All managed care insurers require preauthorization for services. After an appointment is made it is your responsibility to call the number on the back of your card to obtain authorization for treatment.

At the time of intake (your first appointment) you should give this authorization number to the Access Intake Specialist. This will begin your treatment as authorized by your HMO. All subsequent authorizations will be obtained by us. If there is a change in your insurance after the time you begin treatment, you need to let us know so that we can change this information in our system.

We will also need a copy of your new insurance card. Failure to let us know that your insurance has changed or has been dropped will generate a bill to you at full fee.

It is in your best interests to inform us of any insurance changes as soon as possible so that we can set up a sliding fee arrangement with you if necessary.
